Additional Information / Comments:  Please provide further details such as the nature and address of the location if different from above, days and hours the location is open, whether the phone would be accessible to the general public or is private in nature, whether you have an existing payphone contract which you would like to replace and when your agreement expires, and other information which would be helpful in determining the economic potential of your request.  Keep in mind that a payphone needs to make about 100 calls per month to be economically justified without a monthly maintenance fee to cover the $20-$25 per month charge for the line, other operating  costs, plus an amount for capital recovery.  If you think your location will generate less than 100 calls per month, please indicate whether you would still want a payphone if a monthly service charge is required.

*  We retain payphone equipment ownership.  Agency agreement required.  Some sites qualify for free payphone service and a commission.  If your location is outside our current operating area, we will refer you to your state payphone association if one exists, or another reputable independent payphone provider, if known,  to consider payphone service at your site. To comply with the A.D.A., we supply volume control and mount the payphone at the required height - adequate space at the mounting location for wheelchair access is a consideration.
